Prostate problems – from benign prostatic hyperplasia (BPH) to prostate-related urinary symptoms — affect millions of men worldwide. For years, treatment options fell into two categories: medication or traditional surgery. But today, there is a third path emerging rapidly in modern medicine: Interventional Radiology (IR).

Interventional radiology offers minimally invasive, image-guided procedures that treat prostate conditions without large incisions, general anesthesia, or lengthy recovery times. The most widely used IR procedure for prostate enlargement is Prostatic Artery Embolization (PAE).


What Is Prostatic Artery Embolization (PAE)?

PAE is a catheter-based procedure performed by an interventional radiologist.
Using imaging guidance (fluoroscopy or CT), the physician inserts a tiny catheter through an artery in the wrist or groin and guides it to the arteries supplying the prostate.

Once positioned, microscopic particles are injected to reduce blood flow to the prostate. This gradually shrinks the prostate gland, relieving pressure on the urethra and improving urinary symptoms.


Why Interventional Radiology Is Changing Prostate Care

1. Minimally Invasive

No incisions, no stitches, no surgical removal of tissue. Most patients go home the same day.

2. Faster Recovery

Typical return to normal activities: 24–48 hours, compared to weeks after traditional prostate surgery.

Compared with TURP and laser surgery, PAE has a significantly lower risk of retrograde ejaculation and preserves sexual function in most patients.

4. Effective for Large Prostates

Patients with very enlarged prostates (>80–100 ml), who may not qualify for some surgical procedures, can often be treated successfully with PAE.

5. Suitable for High-Risk Patients

Men with cardiac conditions, diabetes, on blood thinners, or unfit for anesthesia often tolerate PAE well.


Who Is PAE For?

PAE is primarily used for:

    • Weak urine flow
    • Frequent nighttime urination
    • Difficulty starting urination
    • Sensation of incomplete bladder emptying
  • Patients who failed or cannot tolerate medications

A full evaluation (MRI or ultrasound, PSA testing, urology consultation) is needed to determine if someone is a good candidate.


What Results Can Patients Expect?

Clinical studies consistently show:

  • 50–70% reduction in urinary symptoms
  • Significant improvement in quality of life
  • Prostate volume reduction of 20–40%
  • Long-term durability for most patients (multi-year data available)

Symptom improvement often begins within days to weeks and continues over months.


Are There Risks?

Although minimally invasive and generally associated with a lower risk of certain complications compared with some surgical procedures, PAE is still a medical procedure.Possible side effects include:

  • Mild pelvic discomfort
  • Urinary frequency for a few days
  • Temporary burning during urination
  • Rarely, non-target embolization

Serious complications are uncommon when performed by trained IR specialists.

Interventional radiology is expanding treatment options for prostate enlargement by offering selected patients a minimally invasive alternative to traditional surgery. PAE is increasingly recognized as an important treatment option for appropriately selected men with BPH, supported by a growing body of clinical evidence and guideline inclusion in many regions.
